Complete the appointment request
When you submit the request, you will be redirected to the online payment page where you will need to make a payment of £20.
Each person named on the ‘Proof of life’ form will need to attend the appointment.
What to bring

proof of identity - a valid passport, driver's licence, bus pass or other photo ID
- Confirmation email of payment
